• Make certain you have the required immunizations
• Upload part 1 and part 2 of the required documents for your team role (.pdf format)
• Send final payment

*If you use PayPal other than for the initial deposit, you must add 3% to your total.

• Submit pdf of medical/dental/ other school diploma
• Submit pdf of state license showing the expiration date
